They need to errata the MM then.
It quite clearly states, under Minion:
"A minion is destroyed when it takes any amount of
damage."
So as written, even if the Minion has temporary hitpoints, it will still die if you do 1hp damage to it, since it took some amount of damage.
It doesn't say that, but it DOES say that minions are always killed when they are hit (unless the damage was done as a special ability that triggers on a miss).

Not only that, but the rules for Stand Up explicitly cover the case where your square is occupied when you stand up. It explicitly says that if your square is occupied, you can Shift one square as part of the Stand Up. This of course prevents OAs even if you have to move.

The pregen rogue for KotSF has the damage for Positioning Strike listed as 2d4+4. But the PHB lists the damage for Positioning Strike as 1[W].
As far as I can see the damage for the dagger would be 1d4, not 2d4...
I think I must be missing something here...
The Keep on the Shadowfell pregen Rogue character has a +8 for their Positioning Strike power.
This is Dex vs Will. The rogue has a dex of 18, for a +4, so where does the other +4 come from?
The same question also applies to the other Dex based...

I think we already established that the Coup de Grace shouldn't have killed the giant (because 1/2 the giants max HP of damage was not done by a single blow).
That's wrong though. Cause Fear allows the feared creature to decide the path that it takes - your example allows the character casting Cause Fear to decide the path.

It follows the Wizard around, so if he sits on it, it will just float there, unmoving. The Wizard must move 5 squares away from it before it will begin to follow.
