In all of my years playing/running RPGs I can’t think of a time a character died “unfairly”.

While I like "Deadly" D&D, IE make good choices and dont just charge into every situation, First thing I did with SD was make it a tad less lethal. Though like most OSR once you get a character to about level 3 its less lethal anyways.
