    You could do a lot worse than use Lulu.com. I've used them for my self-published work, personal projects, and they've always be great. And since you're in the US, the postage should be cheaper too.

    This is a good thread: when's the next installment? I played my first game (as a player) at the weekend, and I liked how combat was a lot more fun than 3.5. Our DM was very much a 'yes' DM, and we were moving a hell of a lot and using all sorts of stunts.

    I agree. This is a great picture of the stirge; looks far more menacing than its predecessors. No doubt the new MM one will be a tougher opponent that the earlier ones too. And if they do end up being more intelligent, so much the better. Makes them more scary in my humble opinion.

    Profession skills and 4th ed

    Don't use a skill for it. Make it part of the characters background instead, and if rolls are needed just use Ability checks. Otherwise, make it a feat, or just make a new skill. Any of those could work.
