    I can picture a well-rested 20th level party of six beating her, but not without heavy casualties- and even then it's not a sure thing. Now, I haven't read the last episode yet, but I expect the characters to face some degree of opposition without a chance to rest before getting to fight Tiamat.

    Shield stacks but Mage Armor doesn't- I suspect Draconic Resilience was put in there so that Sorcerers didn't have to waste a Known Spell for MA.

    Well, the fact that the Tarrasque is either underpowered/overlabeled doesn't mean Tiamat has the same issue. I, for one, really don't care for power creep and over-the-top power level, so as long as adventurers don't go over level 20, CR 30 is not such a bad number to me.

    If I were to run a full-blown grimdark D&D campaign -though I'll probably never will- I'd use the following house rule: on a hit, you lose hit points and gain a level of exhaustion. Just bear in mind: most characters won't last long, and players will try to avoid combat at all cost.

    I've ran an evil campaign exactly once, and it turned out ok because all of the players understood that the idea was to oppose the good guys, not kill each other for the lulz. PCs were the chosen of an assortment of evil deities who had united to mess with the good races of the world.

    When I ran Thundertree, the only enemies in the place were the dragon and the cultists- the rest of the monsters had been killed either by Reidoth or Venomfang. The rest of the encounters were IMO kinda boring, and we were short on time, so I rushed the party to the big challenge right away.
