    Skill challenges (of some sort) The big one is probably banned schools for Wizards (since everyone is a specialist now). More controversially, I'm probably going to require two banned schools for each wizard, one of which MUST be Transmutation or Conjuration.

    If you want to tie it to existing mechanics and explanations, let 1 use of action surge and second wind come back with each Hit Die spent. Since hit die recovery is supposed to represent resting, recharging, and healing, that makes sense in-situ.

    Honestly, I don't think you'd see too much issue with letting Action Surge and Second Wind give a level of fatigue each that goes away after X rounds or minutes. In long combats, it might mean that the fighter got to use Action Surge twice, but I'm totally okay with that. At later levels...

    best AoO build is probably a spiked-chain or Halberd wielding flurry-tripper with combat reflexes and defensive sweep.

    Multiple round attack spell?

    Legion of Sentinels from Players Handbook II. Sorc/Wiz 3, conjurs 12 incorporeal swordsmen with AC 25 and attack bonus = Caster Level. Damge is 1d8+(1/3CasterLevel) per attack per swordsman. Duration is 1round/lvl. It's pretty keen combined with, say, solid fog.
