Of course most fights are well over the party's CR (since in my DM/My opinion fights with CR encounters are actually boring for everyone- and the situation rarely calls for the 3-4 encounters a day...

    Theoretically 100,000 gp per +10, double for everyone past the second for not taking a slot for a total of 1.1 MXP (or Mega=million). Items of that level will probably be addressed in Epic book. Not that I'd know of course.

    Casting a Sleep spell behind a goblin?

    That would be one big tower shield, especially for a goblin. 5'x5' is pretty big, and the tower shield is at best maybe 3' wide (and thats being generous). With a mantlet (or movable wall basically, you could stop it, but thats like saying you can't cast throug a closed door.

    Outsiders get damage reduction?

    I'd call them an outsider with a special clause that allows them to be raised, unlike other outsiders. Wont retype the whole "native outsider" thing from FRCS, but thats the gist of it. Basically allows PCs who are Tieflings and the like to be raised. Otherwise it sure would suck if you died...
