    Dimension door is always the last action of the round per the spell description so the situation you described could not occur. As to whether someone would notice a spell being cast within 50' of them I'd say yes unless they failed a DC 5 listen check.

    Drunken master was designed before the concept of monk progression in a prestige class was developed.     Twinned spells would get sneak attack for each, Split spells would not. A twinned spell is casting a spell twice. Split is one spell with 2 targets. If you get SA damage on partial actions and free actions then you get SA damage for twinned spells.

    I think its a clever use of a typically overlooked 0th level spell. Arcane tricksters don't have the greatest BAB so getting touch attacks is a good use of their abilities. Archers of the same level get off 3-6 attacks per round vs only 1 spell of short range.

    Increasing the number of monsters rather than the difficulty of a single monster is the best way to balance for a large party. A powerful monster is more likely to kill players or be difficult to injure because of advanced immunities.

    A heal 15 check also stops the wounding and outside of combat you can get it in 1-2 rounds especially if you have a masterwork healer's kit. 0 level spells aren't usually in short supply and a 50 gp potion is a whole lot cheaper than a wounding arrow.
