    I should also note that their is an allied NPC who is both deaf and mute, and I though it should be logical that he would be immune to the blasphemy, but after reading through this thread, I'm not really sure.

    No. It is a spread effect, like with a fireball. It fills all available area. So, as a 40-ft. radius spread, it fills the entire room and more than enough of the corridor to hit everyone. And, since the door is open, it spills out into the corridor. It can go around corners.

    I just realized that since the knight is deaf, he cannot be affected by the bugbear cleric's ability. It is a sonic ability, which means the knight would be have to be able to hear the bugbear's words.
