    Some of the most fun I've had in RPGs has come from memorable character deaths. To me there's something a lot more interesting in continuing to tell the story of a character's death than having death completely removed as a failure option.

    Not having the sense to run away from a fight that isn't going well is a problem with the player, not the adventure, the GM, or even whether or not the GM fudges. Unless the GM has cruelly closed all possible escape routes, or blocks them as soon as you get to them.

    These are not the only options: the NPC got good rolls, the PCs assumed they were invincible. Adventuring is an inherently dangerous activity.

    In my early days of GMing, quite a lot. Now, I prefer to roll in the open so players are less likely to complain about bad luck if they can see the roll themselves. In so doing, I'd get caught fudging. As far as I remember, recently, it was only once, in the PBMB game.
