    Yep. We don't use feats in 5e. Never been an issue with old or new players. Chapter in the PHB with feats even mentions in the opening paragraph your DM decides if they are available or not. Players should have no expectations in 5e that all OPTIONAL rules are automatically in play.

    If you really want to go GoT/ASoIaF, then dragon's fire can be extinguished by water, but only if fully submerged. Otherwise, it keeps burning, kinda like napalm (or wildfire in GoT lore).

    Same here. It's never worked that way in the game. The only time simultaneous resolution or whatever would be a thing is in the case of tied initiative (and I think this was mentioned in the 1e PHB or DMG).

    Ah OK. Yeah, it was cool in A&E and sounds similar. In that game firing a pistol was quicker than firing say a shotgun. Reloading each varied as well. Fanfiring was the quickest if I recall but not accurate.
