    Combined player/DM books were par for the course for a good long while, as far back as the 1E Unearthed Arcana, if not earlier. I don't think the tendency to a hard divide between player books and DM books really came into its own until 3E, and even in that edition there were plenty of exceptions.

    Not in my experience. I know several people who 1) play clerics now, who wouldn't prior to 4E, or 2) have always loved clerics, but were ready to abandon them prior to 4E, precisely because they were sick of spending so much of their time in combat healing and doing nothing else.

    Oh, the Stone Thief--the dungeon in Eyes of the Stone Thief--is literally a living dungeon. It travels, it shifts levels around to try to kill the PCs, it builds new levels of itself out of what it consumes, and it actively hunts places and people the PCs love to destroy them.

    Chaosmancer, you'd have to do some conversion, since it's a 13th Age product, but take a look at Eyes of the Stone Thief. I'm reading it right now, as part of my deeper look at mega-dungeons, and I think you might find it exactly what you want, as there's room for both story ideas you mentioned.

    Deck of many things

    I have to admit, I've been tempted to have the PCs face off against a villain or rival in a Deck of Many Things challenge. Each side takes turns drawing a card (or multiple cards), until one side is either dead/gone or decides they aren't willing to risk a further draw.

    A 1 is good on a Divine Intervention check, which is rolled on d%. I think a lot of people responding to this thread missed or have forgotten this fact, and are thinking of the 1 as being on a D20, and being a critical fail.
