    I focus on the campaign themes and flavor and lose focus on the specific adventure opportunities within the campaign. If I could find published adventures that I liked, I’d be happy as a clam, but I’m hyper-critical of my own and others’ adventure design.

    Depictions of orcs and elves can reflect the way we dehumanize other people. We lose nothing by changing those depictions so that people playing the game don't have to look at reflections of their own dehumanization.

    IMO if the player declares that the PC is engaging in sex but does not declare that they’re engaging in unprotected procreative sex, a DM shouldn’t presume it when deciding to resolve the action.

    There very easily can be a difference in expectation about what is common knowledge in the fantasy world. What do PCs know about religions, about factions, about monsters and their proclivities, about geography and history, etc.? The disagreement can be based on those mis-matched assumptions.

    Anyone can use the unclassed NPCs from the Monster Manual as-is. And we have all the rules necessary for building PCs. Anyone can use those to stat up NPCs if they want. Do what works for you.
