    You can. The attack goes off when the target is still on it's initial space. Otherwise you wouldn't be able to attack someone walking straight away from your reach.

    Whack a mole is indeed annoying, but forcing the DM to deliberately finish downed PCs is even worse. Best solution: instant death at 0 HP. No appeal.

    What you describe is just the regular 4e World Axis cosmology. Lots of sourcebooks available for that edition. Shouldn't be hard to use those with 5e.

    I don't see the problem here? Gunpowder artillery have been in use in Europe since early 14th century and by the Mongols long before that. Besides, hard disagree that D&D should be considered "medieval" at this point.

    Just to be clear, I do agree with you on this one. Exploration would be a lot better if we had a bit more crunch to it. It is indeed, underserved as you put it. And yes, the challenge bypassing abilities are badly designed too imo.

    Yes, weather, forced march and diseases are the main source of Exhaustion. I just don't get it why you're taking like it's something super rare to occur. In my current SKT campaign, Exhaustion is always a real threat, except during sessions when the party is hanging out in a civilized area.

    Actually, the combat pillar also suffers from this to some extent. If you stick to the CR guidelines for encounter building, it's virtually impossible to kill PCs with anything that isn't at least double Deadly after level 5.
