As I've written it, Indomitable can apply to Death saves.

I didn't take part in the playtest but IIRC - and someone who was can confirm - Indomitable was at one point a flat Advantage on all saving throws.

One thing you may be forgetting is technology. It requires lesser technology to forge a great axe than a 2H sword. Less steel too. 2H swords may simply not be available. For example, if the campaign is set in Migration-era Europe then there are no 2H swords.

I think you need to think about which spells would already be in effect. For instance the whole place (possibly with holes) should be under a Mordenkainen's Private Sanctum spell. There should be loads of Glyphs of Warding just waiting to be uncovered and activated. Stinking Cloud and Cloudkill...
