From what I gather from the WotC boards, it's the ones from Complete Adventurer that are causing the problems. I tried the 4 page ninja, the 2 and 4 page scout, and the 2 page warlock, and the ninja and both the scout ones gave me the error.

Yes. A die must be "balanced" -- each opposite side, when added together, must equal the die size, plus one. This is often a problem with d20s.
