If that mythos (which I'm not convinced is intended to be in D&D at all) is really that important to you, I suggest each extraplanar creature have a secret True Name which cannot be magically divined, but would allow a caster to Gate in individuals without giving them a choice.

I require my players either have a copy of the PHB or access to the SRD outside of the game so they can properly update their character sheets. During the game, I heartily recommend them owning their own copy so they can look up rules and spells during the game.

Nothing mystical involved... I'd assume blind fight meant the character is good at anticipating a creature's movements based on scant information. I'd allow this, and assume that knowing where it last attacked you from or being able to figure out what part of its body your last blow connected...

This happened a while ago IMC but I just remembered it and wondered what you thought. What happens if you cast Daze on somebody who is swimming? What happens if they're wearing chain mail and have, up to this point, been barely making their swim checks? Do they just sink?

If the target is capable in any way of responding to your attack, even such a simple motion as turning their head or rolling would be enough to not be "helpless" per the rules. Helpless is a very specific condition and very few situations grant it.

Cure and Inflict spells do not have the [Good] or [Evil] descriptors.

I would remove racial bonuses/penalties for mental stats and apply the ones from the new race. This could result in situations where the PC has some stats lowered and none raised. Sucks to be you. At least you're not dead.

If you are capable of calling an outsider by name than I think it should qualify as a unique being. Off-hand, I'd say that only the being named by the caster of the Gate could come through. Of course, the being could disguise itself as an underling and come through, claiming its master sent it...
