    Sounds like you want a game with no dm at all, then. If the dm doesn't make decisions, what, exactly, is his job? To memorize page numbers? Fact is, there can't be a rule for everything that might come up in game. Every rpg has to have the dm make an off the cuff ruling at some point, though...

    Charm Person from 1E: This spell will affect any single person or mammal it is cast upon. The creature then will regard the druid (or magic user) who cast the spell as a trusted friend and ally to be heeded and protected.

    Dragon #96 allows druid/rangers and Dragon #100 addresses the alignment issue. Ranger/Druids must be neutral good.

    Greyhawk ftw. I have never seen the draw of Planescape. I thought it was horrible, plus the vernacular was a huge turn off.

    Death Penalties

    I use this same rule, sometimes. But you're looking at it wrong. Assuming the party is level 5, and the new character comes in at level 4, it's not a penalty, it's a BONUS of 3 levels.

    I've played in probably hundreds of AD&D games for over 30 years, at game stores, at cons, and many, many, MANY different weekly, bi-weekly, and monthly groups, and the odd online game, dating back to the old bbses. Sure, there were house rules, but not once did someone not use spell...

    That's why bless and most other buffs don't have a one round or one round per level duration in 1e. So that result never happens (bless has a 6r duration in 1E.)

    With cyclic initiative, it is very hard to pursue anyone. If two characters have the same movement rate, one can never catch the other until they start failing con checks - barring outside interference, of course.

    That doesn't sound like one done poorly to me. I would absolutely have loved that. While I never like losing a character, that tale would be told around the table for 40 years.

    Simple. Mark the various adventure locales of your choice on a map, toss out a handful of one line rumors and let the pcs go where they wilt.
