For some reason, I always thought that if a paladin commits an act he knows is evil, he can't even atone, and if he commits an evil act, but doesn't know it's evil (a.k.a. miko) he falls but can atone. Anyone know where I got that idea?

As long as somatic components require only arms, I'd say it should require a DC 15 or so swimming check. Treading water with just your legs is doable, but not particularly easy.
