I think a scout/fighter archer build would be pretty fun, and could definitely dish out some heavy damage. Or fighter/psychic warrior; you get a ton of feats that way, and a lot of the psychic warrior's powers work as swift actions or pre-battle buffs.

I'm pretty sure Craft Contingency is inherently broken at high levels, when you can run around with a dozen low level contingencies on at very little cost. It may cost 8 times as much as a scroll, but it's easy to word the trigger such that it doesn't cost an action of any type to activate.

You might want to look at the variant campaign rules in unearthed arcana; particularly the sanity rules.

Those really no rationale for why you can't get AoO on such targets, but I can see how it would be a game balance issue. But the rules are pretty clear on the basic nature of an AoO:

Absolutely not! They can always leverage off the floor. If there's something keeping them off the floor, then they can use that for leverage. It might be somewhat tricky, but that's what the grapple check is for; if it was simply impossible, you can be sure the rules would say so explicitly...

Why not? Simply have it be max(CL, HD), and it'll never break. And casters with missing caster levels tend to be screwed enough as it is; this change could actually make them more viable. This change, along with having all classes increase CL by 1/2, would be a good way to encourage casters...
