    It definitely isn't the case that all spellcasters can do this automatically. I have a half-orc cleric that gets 1 skill point/level, so his concentration check isn't looking so hot. :)

    Multiple slots wouldn't work very well for things like permanency. A simple solution is just to get a reserve pool of XP each level, which can be used for crafting/spell casting. If you wanted to throw non-casters a bone, it could also be used for buying action points.

    I've found the main rules to be mostly ok; but as mentioned above, there's almost no point to resisting grapple when it comes to the larger monsters. (Random thought: having the size bonus only apply on offensive grapple checks might kind of fix this.)

    A lot of epic feats are substantially worse than the upper level stances. (A lot of epic feats are hideously underpowered in any case.) I'd say that restricting it to light melee weapons makes it pretty balanced, considering the other stances out there.

    That's actually a horrible hack. If the monk/soulknife loses the 2pp granted by the feat, they can no longer obtain psionic focus! And there are several ways to cause pp damage. It should just be explicitly stated that the class can gain psionic focus, if it's needed for class features. :)

    Eh, an easy houserule could be to give two fractions of each large group a +/- 5 vs. each roll. I'm thinking of the DM just eyeballing the numbers in each group, not a complete set of complicated rules for determining exactly which monsters get the bonus/penalty.
