    Yeah, I really like the idea of bursting through the floor. Problem is, he was in a dungeon, hitting the floor with no open space beneath it, so at the very worst it would've just made a small crater.

    Well you can't get complete bed rest if you fight. I'd rule for the day or night thing that someone would have to spend at least 24 hours to get one set of 2/level in Hit points healed for comp. bed rest.
