    I love the idea of an "encounter" book, which would include not just monsters but traps, hazards, social stuff, etc. Perhaps the topic is too broad for a single book, but it definitely makes sense to place all of that stuff in one spot for the DM.

    I believe that there are demon minions with this ability, from which one could infer that it triggers before the damage. Otherwise, the minion would never get to use it. But that's hardly ironclad proof of how the timing should work.

    I would say keep weapons with plusses, at least. And maybe armor, but honestly AC ceases to really matter much past the mid-levels, unless you're seriously optimizing for it. Those types of items were traditionally where non-spellcasters got their combat power from.
