    You're right. If you want to do more than hold them in place you'll need to restrain them. Have your character bring manacles or rope along, and tell the GM how you're restraining the opponent. With luck he'll check out page 42, get a damage rating and DC, then tell you to roll the dice.

    You subtract the threshold from the bonus of the item, not from the NPC. Since bonuses are never negative, you can't hurt the NPC by giving him gear.

    Definitely have them use it. It may not matter if it doesn't overcome their magic threshold, but it doesn't make sense for someone who owns a powerful items to leave it in their pocket so their killers can benefit. And they don't know they have an item threshold.

    The Drake Swarms are crazy. There's also the Yuan-ti with the dominate that recharges on a 3+. It's to hit bonus vs. Will is higher than other 17th level monsters have when attacking AC.

    When you're tied up, hit by an effect that says it restrains you, or whenever the GM feels it appropriate (for instance, I'd probably rule that someone grabbed by one person per limb was restrained instead of immobilized).

    Only a few of them care about your surges. The rest have other triggers. All magical tattoos react to the character's inborn abilities: surges, being bloodied, etc. Presumably it's because they're more than just an item, they're tied directly into your nervous system.
