The Fort DC for choke hold : 10 + 1/2 level + Wis mod. Since most using characters are monks with high Wis scores, this is a high DC, especially when uses vs enemy casters.

As far as I know there is no official rule. We use the houserule that you can't apply your dex to your reflex, since you also lose your dex to AC. Automatic failure seems far to powerfull to me. You can always try to use the other grappler as a shield vs the blast ;)

CotV doesn't really stack with anything, its just that when you have an unarmed damage method that does more than d8 dmg, that gets healed.
