In any level-based game that I DM/GM, everyone starts at first level. Period. I have never made an exception to that. I always make sure that the early adventures are suitable for that level, so they are no more vulnerable at that point than they are at higher levels.

Exactly. If there are no consequences to the decisions they make, they will continue to make poor decisions.

I would have the bad guy do whatever is in character for the bad guy to do. If that means killing the character who is mouthing off, so be it. I don't coddle players. Their choices need to have consequences.

When I was in my teens and 20s (back in the 80s and early 90s), they frequently got that high, because I had lots of free time to game and lots of friends who were into it. These days I have trouble finding people to play with, much less people who will stay in it for the long haul.

I have found that it's a good idea to set player expectations before a campaign. In fact, I usually do so before they have even agreed to be players in the game. I give them a good rundown of my GM/DM style, what I reward, what the world is like, what to expect in terms of general danger...
