    My group- never. And we started pre- PHB. I don't recall any other groups using them either at the various after-school clubs and library clubs I was a part of, nor later into the 1980s. That said we just used any rulebook as guidelines and did our own thing.

    I'm not sure if you are arguing for the sake of it, or what- but I state he was also "off his game", not just exhausted. HP in D&D have always represented a combination of factors- it's an abstraction. My proposed system is no different. If you don't like it, that's OK.

    Yep that's a case where the points were exchanged- Maybe Lay on Hands could be giving the wounded character another death save attempt - The Paladin would "pay" for it with their own HP.
