I am thinking about calling my fantasy heartbreaker "Edition War" instead of RCFG. Then lots of people will talk about it, and lots of people will see it, even when it isn't there! :D
I use a combination of random roll and simply selecting, based upon what makes sense to me when devising an area, and what I feel like at the time. If I have a clear idea, I go with my gut. If I do not, I roll.

While I'm not sure about the rules (specifically, how long chart look-up will take in actual play), I do know this -- I haven't had as much fun reading a rpg book since Gygax left TSR.
