Damage save in M&M uses a fixed number for damage, which is added to a base of 15 to get the DC for the save. It's wonderful simply because it reduces rolling. ;) I wouldn't use anything that would force me to roll more unnecessarily. As for the division by 5, that's just like in the M&M FAQ...

Yeah, thankfully they're not playing the Return to the Temple of the Elemental Evil, but the original one by EGG (modules T1 to T4). Thus, they MIGHT survive a bit longer. But I wouldn't count on it. ;)

The attacks need to be natural attacks to be eligible for Multiattack, so that doesn't apply to this particular situation. A 4-armed character would indeed be able to use 2 greatswords, but would only get 1.5x str mod on the first sword, and then 1/2x str mod on the second (off-hands)...
