Yes, for those 2; I think it is 4th level, but the rest don't. For the OP, either the extra familiar feat or obtain familiar feat should work fine for you. I don't see how the 2nd familiar will unbalance anything. Would be fun to watch it in play.

The DM may have used the WotC update of White Plume Mountain for 3.5, but not sure. It's here: Adventures: White Plume Mountain (Revised) I haven't downloaded it, but the text on the page says the weapons were modified/updated to be Legacy weapons.
