I think you're confusing players with characters and narrative with narration. The very act of playing an RPG is a process of constructing a narrative. Whether or not that is important to the players is, of course, debatable.

I'd argue a character could conceptualise their skill in their chosen profession in a similar fashion.

I didn't think you were :) Don't get me wrong, I'm a big fan of CoC and have no qualms about the sanity rules. I personally think there should be more of this kind of thing in RPGs.

Rules for morale certainly exist in 1e. They seem to have dropped off the radar somewhere between then and Pathfinder.
