Found these threads to be useful for my own game. (Excuse the references to Eberron areas, thats where my game is set)

1) The lava pits below Sharn. Much like Revenge of the Sith, the players must manuever on low flying harvesters over the lava.

2) An airship flying near the Floating Towers of Arcanix, the combatants must be able to fly, be on flying mounts, or be very careful not to fall off the ship.

3) inside one of those ridiculously colossal++ epic monsters

4) Freefalling through the Elemental Plane of Air when none or only a few of the combatants can fly

5) On a sheer face of rockface against creatures with a climb speed

You forgot the iconic Eberron fight location:

3) A speeding Orien train, with the battle occurring inside the cars, on the roof and on the backs of the mounts being used by the attackers.

"Oh no! Thog fell off the wagon!"

santa claus style. in a chimney. losing dex due to the tight space. losing reflex saves too.

a fireball/lightning bolt wizard at the top. and some guy lighting the fire at the bottom.

On the bodies of two fighting gods of unimaginable sizes; as the gods hit each other tiny, almost insignificant followers of one leap onto to the other where his followers must repel the invasion.

In a dungeon; during an earthquake; as the entire complex is falling on your heads.

Inside a slowly collapsing tower.

On a sinking ship
A narrow cliff path
A treacherous crevasse filled glacier
A room filled with traps. (the first trap releases monsters on stasis. Now the pcs have to fight in a room where any further movement might set off more traps.)
Caverns inside an active volcano that (naturally) is about to erupt.

In a 10' x 10' room with frost giants.

...Or not.

Actually, Dungeon #100 had an adventure called "Beast of Burden" that involved an army of gnolls in towers being carted around on the back of a colossal creature, that I thought was very cool. Sorta like a kaiju mumakil. I can imagine lots of cool "swinging on ropes from tower to tower, getting bashed by passing treetops" type action...
