If I were to run such a campaign, here's what I'd do:
Basics: Scout, Knight, Favored Soul, Duskblade, Spirit Shaman, Beguiler, Dragon Shaman, Warmage, Warblade
Psionics: Psion, Psychic Warrior, Wilder, Erudite, Lurk, Ardent
Wierdos: Binder, Shadowcaster (with Ari's fixes), Truenamer, Dread Necromancer, Warlock, Spellthief (enhanced in some way, it's a cool class but kinda weak)
Psionics and Magic would both be accepted as normal, and people would be more used to limited spell list spellcasters -- a high-level Favored Soul may not have restoration, so the PCs may have to quest for one specific FS who can actually cast that one spell they need.
Likewise for "advanced learning" or whatever for Duskblades, Beguilers and Warmagi -- some will know particularly cool "extra" spells.
I'd allow the Extra Spell feat to grant any spell from any list, since all lists are so limited.
