D&D 3E/3.5 3.5 polymorph

Can anyone explain (or point to something that does explain) exactly what of your own abilities you lose when you polymorph? If you have an [ex] ability due to a feat, does that get lost (as it is not from class levels per se). Does it matter of the ex ability is an attack?

For such a commonly used spell this thing seems like a mess. I know we've had 3 iterations of it so far (3.0, Tome and Blood, and 3.5) but it still seems like it needs a lot of help.
