"Racial Hit Die" refer to the hit die of a race. If you slap the half-dragon template on a 4th-level fighter/Ogre, then your half-dragon ogre has 4 racial hit die and 4 class hit die. Thus, it's breath weapon is based on 4 hit die (that of the ogre), and it would gain skill points based on each separate set of hit die.
