4 counterspelling questions

You're quite right of course. Quickened spells used to provoke in 3.0, but don't anymore. I've edited my post above to fix the error.

I'm away from my 3E PHB. But the 3E SRD I'm looking at says "Cast a Quickened Spell [Free] [AoO: No]"...

2. WOTC CustServ said "No" when I asked. They give routinely messed-up answers, so it's not a great source. I think the answer should be "Yes" when using dispel magic.

Put it this way, it's certainly not a problem to allow it for dispel magic. Counterspelling an item's spell will always be an inferior tactic to just dispelling the item. It's at best the same caster level check (worse in cases such as when the user of a staff of fire is higher level) and it still leaves the item functional.
