Really great! Two corrections:

The "attack type/modifier" table is incorrect. Ranged attacks do NOT take a penalty against prone targets. The defender gets a defense bonus, which is correctly described under the prone condition.

It should also be noted somewhere that running gives a -5 penalty to attacks until the start of the character's next turn. I tend to forget about this one, and it's important.

A fair warning though

I couldn't find an updated version so I created my own with a few more details, updated rules and a 2nd page for Conditions.

Kudos go to Kiznet for the original.

V1.4

Edit: Updated to v1.4 inc the May rules update (Aid another, Aid Attacker, Aid Defender and Conditions.

If you happen to have a somewhat finicky printer like mine (an HP Laserjet 2030 styled printer), trying to print out the 1.4 cribsheet might result in the printer spooler getting all messed up. It took a fix from this webpage:
You cannot add a printer and you receive printer spooler error messages in Windows XP to get things fixed up for my printer. So fair warning to those out there.
