I did the following with the 3.0 and later 3.5 PHBs:
For small corrections, I would use white-out tape, and a very fine-point bleed-proof, waterproof pen to replace relevant text.
For larger sections of errata, I printed them out, and then closely cropped it down to just the necessary text. Using two small pieces of scotch tape, I would create a small hinge for each of the small cut-out errata pieces, and place them in the appropriate spot. Essentially, I tape the tape to the book, and not the paper to the book.
For other books, if the entire errata was one page or less, I would just tape the page into the back cover of the book, and mark with asterisks in the appropriate sections to tell me I should look at my errata page.
