D&D 5E A bit of flavor... and afflictions for fun.

Mostly the idea is about fun flavor.

Basically there are a number of powers which create temporary effects, but which also deliver damage one example is the Wizard/Witch power called beastswitch. If a player defeated an enemy using that power... it is badass if the "dead" enemy is simply permanently well a mostly innocuous beast as per the flavor text. This could be undone with a level 8 ritual of course (just as a raise the dead ritual could undo the bland dead enemy interpretation. Or the victims mind might be stuck as a field mice after that psychic damage even if their form is not.

Note the remove affliction is the same level as raise the dead so an effect in game that creates an affliction such as the above would need to be well of "killer" intensity.

Just a thought.
