A poor death for a warrior

Please fill us all in on the poor deaths PC's have seen.

No brave last stands or hideously planned traps. I wanna hear about 12th lvl PC's freezoing to death, falling to Cyrulean flu or choking on chicken bones.

It all started when the DM said, "Hey guys, I just picked up this great adventure module called, The Return to the Temple of Elemental Evil! you guys want to play it?"

Like fools, we agreed.

Commence eight months of pitifully meaningless Total Party Kills.
