a swarm lycanthrope?

Since a swarm is assumed to be one animal, for the purposes of saving throws, etc., can a small humanoid then become a lycanthrope whose alternate form is comprised of 300 tiny fish?

Not sure what you're asking. Is there something in the rules, you mean, preventing you from creating such a creature? Heck no! It's a fine idea for a monster.

Incidentally, to see a zombie swarm or two, check out the ongoing Iron DM Tournament, in which the current round (Rune vs. Macbeth) contains a zombie swarm as one of the ingredients.

Or worms or bugs. Its...The Oogy-Boogy Man from Nightmare before Christmas! :)
 

Now now. Oogy Boogy was clearly a Worm that Walks.

But yeah, a swarm lycanthrope sounds friggin' AWESOME. Would it have a hybrid form? Could it transmit the disease?

There is already a swarm PC in the game, a Fine Damselfish Hivemind (Swarm, Aquatic). Now, if that PC was a lycanthrope as well, perhaps it has a hybrid form it has never discovered. A can of sardines, perhaps? ;)

Wow! I just have to use a Half-Fiend Lycanthrope (Rat Swarm) now as a sinister bad guy in my upcoming new urban campaign set in the Wizard's Reach! That's great! The rat man cometh!

Would those RATS happen to come in a BAG, by any chance?
 

Dracula/Gary Oldman in the 1992 "Dracula" turned into a swarm of rats, and I think the SFX were pretty good, although it's been awhile since I've seen it. There was also a demonic assassin in an episode of "Buffy" who was a big pile of insects (or was he worms? I can't remember now).
