OK, got my KotS.

1. What does Recharge 5 6 mean for a monster's special attack? Is it the number of rounds? is it a dice roll?

2. What is the effect of being "Slowed" by a flail?

3. WHat are those little SWORDS next to the monsters attacks. Circled sword and regular sword.

Before you do anything at the beginning of the monsters turn you roll a d6 and lands on one of the recharge numbers it is recharged. If it doesn't you can't use that power till it does land (whoever many turns that may take).

Slowed is right in the book on page 13: Essentially your speed becomes 2 for all normal travel, so not teleportation or forced movements.
 

The circled sword is the Basic Attack. Its the monsters standard attack and the one it uses when charging, taking OAs and the like. The sword in general denotes a melee attack ability, as opposed to the :ranged: ranged tag.
 

:5: :6: means it recharges on a d6 roll of five or six
:bmelee: basic melee attack for charges, OAs, extra attacks granted by other classes powers etc
:melee: a melee attack, NOT basic
:ranged: ranged attack
:area: area attack
:close: close attack
