Aid another in attacks?

Something's seems a little wonky here. You could have 7 people adjacent and threatening, plus another 16 with reach weapons, plus another ??? with reach weapons of 15 feet, etc., etc., etc.. Even with only the first and second rank, that's still 23 people (or potentially +46!!!).

And don't get me started on kobolds with reach weapons!!!

Personally, I'd be much more afraid of 23 attacks (keeping in mind that that's 23 chances to roll a natural 20) than one attack that's bound to hit.

Also, there aren't any medium-sized reach weapons with 15 feet of reach. And if you're dealing with giants, that's just getting ridiculous and doesn't even apply.

... no. Rats have a 0' reach, so they can't threaten the giant rat's target.

As Tiny creatures, you can fit four in a square, and they have to be in their opponent's square to make a melee attack... I'm not sure if you can fit four Tiny creatures and a Medium creature they're attacking in the same square, but that's certainly the upper limit on how many can Aid the giant rat.

Here's a twist on the question. If "Mook A" uses aid another, does that +2 apply to all the attack "Badguy B" gets or does it apply only to the first attack?

If it applied to all, that makes it much better, for example...

Mook A = +1 Melee (slap) 1D2-1 subdual
Badguy B = +11/+6/+1 (Greatsword of badness) 2D6+25

So is "Mook A" aids "Madguy B", I, as DM, am giving up a useless attack and getting +2/+2/+2 on a big one. Cool, but does it work?

"You make an attack roll against AC 10. If you succeed, your friend gains either a +2 bonus on his next attack roll against that opponent or a +2 bonus to AC against that opponent’s next attack (your choice), as long as that attack comes before the beginning of your next turn."
