Calvin Quicklift: Basic D&D Thief. First character to die in a game I DM'ed (paralyzed by Carron Crawler and subsequently consumed).
Needledick Buckfucker: AD&D Human Fighter. We were 15 years old.. what can I say.
Snurd Pennystealer: Half-Orc Assassin.

Picture it, I am nine years old in the 80s and trying to teach a bunch of kids how to play D&D. The one kid creates a character and when I ask him his characters name, he replies with:

Ding Dong Farty Marty

In the decades since, that has never been topped.

But seriously, I always find it fascinating what names players come up with. Like, we've all got these linguistic ideas floating around our heads from various novels, movies, series, and real life that coalesce in different ways when it's time to answer the seemingly-simple-but-not question of "what is your name?"
