Anyone plays with instant kill?

One of the things I've done imc to make it worth taking fighter levels above fourth is to add a few more "fighter only" feats. One of these is Lethality, which is roughly based on the instant-death option (a threat followed by a natural 20 means the victim must make a Fort save or die, DC is 10+dmg dealt iirc, but I don't have my stuff at the moment).

I've also added a Weapon Mastery feat which gives a +2 (unnamed) bonus that stacks with everything that can be applied to attack, damage, or split between them (the fighter can change how it's allocated with every hit).

On page 64 of the DMG, there is a variant of the critical hit rules that goes like this:

- On a natural 20 o an attack roll, a critical roll is made to see if a critical hit is scored. If that critical roll is also a natural 20, it is considered a threat for an instant kill. Now a third roll is made, if that roll scores a hit on the target, the target is instantly slain.

Anyone uses this variant? Any interresting stories regarding it?

Had a rouge in my group take down a nightmare beast with one arrow. (I did not see that coming). I think of it as the one grand strike that lands the player a kill not based off of the enemies weakness but shear luck/skill for that attack.

I decided against it for my campaign, but when I was looking at it, the progression I was considering was hit / crit / coup-de-grace.

I use the rule as well, at the player's request. Told them that they can do it to the monsters but the monsters can do it to them.

Killed two bad guys and one PC with it. My group likes that kind of stuff - they are the 'roll everything in the open/pull no punches' camp too.
