Some already have been errata'ed, such as blade cascade, but what was shocking was that people were already pointing it out the day 4e PHB was made available. These weren't combos that required an intimate knowledge of the rules and/or outright system mastery to catch on to, anyone with a rudimentary grasp of 4e rules were already realizing just what could be done.
Other examples:
Sehahine's reversal - can you reverse the death status? What happens then?
Orb mastery, spell focus and high wis - auto-lockdown 1 normal foe/encounter at epic lvs.
Demigod's divine miracle and Archmage's archspell, though it is debatable how much they can upset game balance when they are available only at lv30, when your campaign is nearing its zenith.
Seal of binding+regeneration. Just very poor design.
Fighter's rain of blows. If you hit with the first 2 hits, you get to make 2 more attacks, for a total of 4. Which nets you more damage than any other fghter encounter3 power.
Warlord's relentless assault. As written, stacks with itself.
Bloodmage's blood pulse/bolsering blood. Weird wording, and cust-serv just made it worse (Orcus takes 16d6 damage for each square he moves?!?)
Vague rules on forced teleportation - by RAW, you can teleport a foe into the air. Fun comes when the enemy wizard does the same and drops your PC into a very deep pit. Not sure if it is intended though.
Ranger's follow-up blow (lv29 daily) can theoretically trigger itself. It is possibly another blade cascade fiasco in the making.
Twin blade+2 weapon flurry+heavy blade op. Still the subject of much discussion, but another possible avenue of infinite attacks depending on how you interpret the rules on AoOs.
More are probably just waiting to be discovered...
