On my terms, though that's not the most accurate description of how I play. I don't massively house rule, because IME, it's a PITA. I generally tend to ignore details to rules that aren't really worth enforcing. So for example, I don't pay a great deal of attention to encumbrance. I tend to ignore AoOs and similar rules when not using minis, because they're a pain to rule on without the visual aids. I tend to make a lot of on-the-fly rulings as was typical in earlier version of D&D, but I don't do it as often as I did pre 3e, because the 3e skill system and saves are good enough that I can use them without having to wing it.

The Doctor is as Anomalous as ever. For reasons too stupid to go into (well, OK... A store discount card got curled in the wash), I have all my D20 material on the table right now. Standing several feet of card-flattening magnificence high, I do beleive we use each and every one of them (including feats and systems from Star Wars, Mutants & Masterminds, Spycraft, and even Silver Age Sentinels D20) as legal character options. The whole list of Unearthed Arcana rules and variants I allow are listed in my PbP ooc thread.
