I have all new races. I only kept three of the core classes (and incorporated about five others) and I've replaced the magic system. I've got special armor rules, AC class progression, and other tweaks to the gameplay as well.

I'm a self-proclaimed "d20 fantasy" as opposed to D&D GM. ;)

I use the rules, not let the rules use me. ;)

I use lots of options. But I don't really think character options using existing rules (feats, prestige classes) and new rules are the same thing. Your poll conflates the two, so I really can't answer it.

I stick pretty close to the RAW, though allow logic to trump it, i.e., I don't feel at all bashful about extending rules on the fly. But I try to use the existing structure, which the 3.xe system is very accommodating about.
I have a few house rules. Not many are required since I am addressing primarily taste issues (like making injuries more detailed). In earleir editions, my house rules were much more extensive because they were more corrections.

I use a very small number of house rules. Most are only related to character generation. I prefer to work within the rules as it allows any player to join in my game and easily know how things are going to work.

If one does use a lot of house rules I just like to see them consistantly applied and documented.

I use the core rules and the campaign setting specific rules for the most part, but I have no problem houseruling something I think is broken or I do not like or I think will upset the balance too much. I think I am in-between the 2nd and 3rd choices on the poll.

I work hard with any set of rules to beat them into submission so that they fit the game, rather than the game fitting the rules.

Option #3 for me, please

d20 Fantasy all the way!

I'm using Elemts of Magic revised, Ken Hood's 3.3 Grim 'n Gritty rules, Seasong's YAARGH Armor rules, modified classes, new feats, and the rotting carcass of the 3.5 RAW. Butchered that up pretty nicely. My characters don't even resemble Core characters anymore, and that's how I like it.

Lots of non-core options used such as feats, prestige classes, spells, items, and monsters.

A few minor house rules, dodge is a flat bonus, polymorph/wildshape was reworked, I stopped rolling most social skill checks and just adjudicated without dice or referring to the skill guidelines, etc. Generally stick to RAW for most everything.
