I don't particularly like killing PCs, but I also don't want players thinking that I won't. If they're asking for it, they're toast. The player that seems to die the most is the casual gamer who just likes to pick fights, and won't ever back down -- unsurprisingly she's had to roll up new characters more than once.
I also don't typically allow dead to be raised. Then again, I typically don't play with the standard D&D magic.
