Continuous. It started a bit more episodic at the beginning but after nearly 7 years, the plot is closer to "24" than "Buffy." Time is critical. There are so many irons in the fire that the party assembled a 23-item "to do" list , of which only 5 were rated as "unlikely" to cause a character's death. There were almost as many marked with the "potential TPK" death skull symbol.

And I'll note that the 23 are the ones that the players identified, this doesn't even cover the as-yet-unrevealed DM plots.

Mine are a bit of both. I plan them as being episodic, and what doesn't finish in one session picks up with the next. Although most of my episodes tend to flow together in a generally continous manner.
 

My games are about 50/50 ... episodic adventures (though not games, rarely do my adventures fit in a single session) often with an over arching plot linking them together.
 

Now, as an adult with a full-time job and actual resonsibility to Get Things Done, my games tend to be episodic. Then, as a college drop out with enabling parents and scads of free time, I played in and ran a great many continuous campaigns. While I miss those campaigns (and that free time), I rather like being a productive member of society ;)
