I myself plan on punting on the issue -- every appearance of "death" in the books will be replaced with "grievous wound", whose effect is to cause immediate passing out, followed by a month or two of bedrest and healing, preventing adventuring and most activity.
Thus, a permanent wound will be represented by being "killed" via the roll-three-times-to-bleed-out system. It can be healed via the rituals which usually raise the dead in the core system. Failing that, a player may remove one of the three death tokens they're still carrying with a save, one save per month.
